I’m pretty sure the canon is that their dicks are all genetically removed by the emperor already, and they also receive a massive dose of medical-grade hormones to undergo a massive physical change. One could argue that all space marine stories are already trans.
There’s nothing that says they don’t have prosthetic technodick replacements that can withstand all the gay sex their flesh couldn’t.
